Online Services

Online services offer convenience and an enhanced patient experience, all through a secure Web site. Patients can use online services* to:

  • Obtain general information about the practice
  • Refill medications
  • Schedule appointments
  • Review their medical record
  • Enter home-monitored medical data—such as blood pressure, blood sugar and weight—directly into their medical record
  • Conduct online e-visits with a physician (fee of $25)

Access to online services is limited to established patients of this practice. Select an option below:

Prescription Refills

Phone request for refills of routine medication may be left on voice mail by calling our office and choosing the prescription refill option. With your request, please be prepared to leave the following information:

  • Your name
  • Your birth date
  • Prescription name and spelling
  • Dosage
  • Pharmacy name
  • Pharmacy phone number, and phone number where you can be reached

Then call your pharmacy first to determine if your prescription has been phoned in by our office. Messages left after 3:00 p.m. and on Saturday and Sunday are answered the next business day.

Fees And Payments

Payment is expected at the time of your visit unless you have Medicare or another participating plan. Please check your provider book to determine if we are participating providers in your health insurance program. Copayments are due at time of service. This is required by your insurance contract.

Our billing office will make every attempt to assist you with your account. If you have questions about your account, please call our office.

It is very important for you to understand your insurance coverage. Reading through your plan before your visit will prevent misunderstandings.

Appointments

We see patients on an appointment basis and we request that you call in advance so that we can reserve time for you. When you call for an appointment, our staff will ask you the follwoing questions:

  • Name
  • Reason for your call, problem or concern
  • Telephone number where you can be reached
  • Date of birth

New patients should arrive 15 minutes before appointment time to fill out medical history and other necessary forms. If you are unable to arrive early, it may be necessary to reschedule your appointment.

We try our best to stay on schedule, although emergencies sometimes arise. If we are significantly delayed, we will try to notify you beforehand. Please assist us by being on time for you appointment.

If you have any changes in insurance, address or other information, please arrive early. Patients who do so, assist in maintaining patient flow and provider schedules.

Occasionally, patients in the waiting room may be seen out of order if they are urgent. We ask your patience if you have to wait. We will show you the same concern when you are seen.

If you find that it is impossible for you to keep your appointment, we ask that you inform us of the cancellation at least 24 hours in advance. This courtesy allows us to be of service to other patients.
